A performance-based referral is a formal way for a supervisor or HR to connect an employee with Canopy EAP when there are concerns about job performance, attendance, or conduct.

The goal is to support the employee’s success at work and help them meet the performance expectations set by the employer.

Most employees access the EAP on a voluntary basis. In some situations, a formal performance-based referral provides an added layer of structure and accountability while still focusing on support and problem solving.

How to Get Support:

If you would like to consult about whether a performance-based referral is appropriate or need help with the paperwork, contact Canopy at 800-433-2320 or send a secure email.

Submit a Performance-Based Referral:

  • Complete the Performance-Based Referral paperwork for the employee.
  • Use our secure email portal to send the paperwork:
    o Address your message to pbr@canopywell.com
       and attach the completed forms.

What Happens Next:

  • The Performance-Based Referral inbox is monitored Monday–Thursday, 8:00 am–5:00 pm PT, and Friday, 8:00 am–4:00 pm PT.
  • A Case Manager will confirm receipt of the referral once it has been uploaded and reviewed.
  • If you do not receive confirmation within one business day, please email pbr@canopywell.com or call 800-433-2320 for assistance.
